To help us prepare for a safe and successful school year, please complete the forms below electronically.
Climbing Wall Permission Form (One form per student)
Every student must have a completed permission form on file before using our climbing wall during PE or other school activities.
Student Device Agreement (One form per student)
This agreement must be completed for each student who will use a district-issued device throughout the school year.
Free & Reduced-Price Meal Benefits Application (One application per family)
Please complete this application even if you do not believe your family qualifies. This form is incredibly important for every WESA family.

Student Summary Forms
To ensure we have hard copies of student information available in the event we lose access to our computers, Student Summary Forms will be sent home on the first day of school.
Please:
- Review the information carefully.
- Sign the form.
- Return it to the school office by Friday, August 14th.
If any information needs to be updated, please write the corrections directly on the form before returning it.
Student Health Forms
Please complete your student's health forms and return them to your child's teacher by Friday, August 14th.
